Further Details:

The professional relationship between a patient and psychiatrist requires guidelines in order to protect both parties involved. It is important to set up clear expectations from the start of treatment.

Payment and Invoices: All payments are by credit card which will be charged after each visit. Invoices are supplied at the end of each month which include all charges made that month

Cancellations: At least 72 business day hours are needed to avoid being charged the full fee

Frequency of Visits: Frequency of visits depends on an individual’s clinical needs. New patients are seen at least monthly for the first six months and never less than quarterly.

Emergency: A psychiatric emergency should be handled by calling 911 or going to your nearest emergency room. Please provide your physician with my contact information, and I will make every attempt to call them as soon as possible.

After Hours: Any e-mails or phone calls made during business hours will be returned within 24-72 hours unless I am on vacation. In order to maintain a healthy work-life balance, e-mails will only be checked once a day to look for urgent requests. All non-urgent matters will be handled during business hours.

Urgent Appointments: If you need to see me urgently and can’t find an appointment, e-mail me with your available times, and I will do my best to set up an office visit or phone visit as soon as possible.

E-mails: Only non-clinical information should be discussed via e-mail. This would include coordinating appointments or basic information. Topics such as response to medication, treatment recommendations, and side effects should be discussed during appointment times.

Medication Refills: Any medication refills will be discussed during appointment times. Any concerns regarding getting prescriptions filled should not be sent via e-mail.

Controlled Substances: Medications which are labeled as controlled substances require at least monthly visits. More frequent visits may be necessary. Refills of controlled substances cannot be filled early under any circumstances. Medications should never be shared with others and a urine toxicology may be required if I believe it to be clinically necessary.

Forms and Letters: Filling out extensive forms for administrative purposes, work, school, or disability will be charged at the rate of $100 per 15 minutes if more than 15 minutes is required.

Phone Consultations and Collateral Care: Extensive phone conversations with therapists, family members, partners, or children will be charged at the rate of $100 per 15 minutes if more than 15 minutes is required.

Release of Information: By engaging in care with me, you are giving me permission to speak to any other clinical providers you are seeing for treatment. I will notify you before I do this. Specific permission in the form of written authorization is needed to speak to family and/or friends in non-emergent situations.

Discontinuation of Services: If either of us decides to end our time working together, at least a month’s supply of medication and referrals will be provided if requested. I reserve the right to end treatment if I believe our therapeutic relationship will not be beneficial to your care.

Social Media and Public Interactions: Because of the nature, privacy, and ethics around psychiatric care, outside connections on social media are not advisable. If we were to run into each other in a public setting, I am not allowed to acknowledge you without you acknowledging me first to maintain your confidentiality. You are welcome to bring up concerns about this during our sessions.
